Talent Work Sheet

About the Talent Work Sheet:

The Talent Work Sheet is a required part of the casting Process.
It should be completed  COMPLETELY and PROPERLY.

Those of us that are in the business of processing the talent's paperwork, for those booked, spend numerous hours (behind the scenes) making sure that the process is successful.

The information contained thereon is used by the production company, SAG, wardrobe and the Casting Director to fulfill all requirements (some of which are legal requirements of SAG).

Even SAG talent should provide Social Security  (or SAG number) which is required to be furnished to the SAG regional office for all "Booked Talent".

DiPrima Casting computers use the Year of Birth to quarry talent searches.  The Date of Birth is the ONLY CONSTANT FIGURE which remains unchanged through out life.  The computer is set to quarry Year of Birth 10 years, after which the Casting Directors select, to their best judgment, the right person for the role in question.  Talent are encouraged, but not required to input the correct Year of Birth.

It is to the talent's advantage to provide correct information.

NOTE:  It has been my personal experience to witness Production personal tear up (disregard) Talent Work Sheet which were unreadable or improperly filled in.

With these facts in mind, I have elected to make available the
Talent Work Sheet in advance.  Those individuals who wish to 
save time during the casting will elect to download this form .  Remember NEATNESS COUNTS.  I suggest you fill the form out completely, then make photocopies for future castings - the form never changes and is always required at DiPrima auditions.
